HC death after killing Sky Seer?

I was doing trial of chaos on my level 43 char. 600 health 1800 ES, almost max resists. Sky seer died, and then a second later I degened super fast and died. Like within a second. What causes this? Guess I’ll be ready to pause immediately after killing the sky seer now.
