Who Actually Starts Instagram Trends? And Why Do So Many Reels Have Weird Captions and Random Stickers?

This is something I've been wondering about for a while.
Instagram seems to have a new trend every few weeks. Recently I've been seeing things like Korean-style edits, videos being turned into game intros, certain editing styles, specific audios, and other reel formats that suddenly appear everywhere.
What I'm curious about is: where do these trends actually come from?
Are there creators, agencies, or marketing teams intentionally pushing these trends? Or does someone randomly post something, it performs well, and then thousands of people start copying it?
Another thing I've noticed is that a lot of reels have content that doesn't seem to match the caption at all.
For example, the reel might be about one topic, but the caption is talking about something completely different. I've seen this across hundreds or even thousands of reels, so it feels intentional rather than a mistake.
I've also noticed random GIFs, stickers, emojis, or visual elements being added to videos for no obvious reason. Sometimes they don't seem related to the content at all.
Is there a strategy behind this? Is it related to clipping, retention, SEO, engagement, or something else?
