Help me craft this nicely rolled talisman

I haven't dabbled with crafting much, some desecration and perfect orbs is about it. I assume I want to get rid of the Cold Damage first since it's a T6?

I usually don't come across drops like this, so not sure what to do with it. With the right physical rolls, it could actually replace my current weapon if I choose to keep it. Anyway, looking for some guidance on crafting it.

No matter what you do, it's going to be expensive and you'll need to use (at the very least) Sinistral omens to Annul the prefixes only. You'll likely need to perfect exalt slam/prefix annul until you hit T1 Phys%, then you'll have to get lucky to hit flat phys either with more slams, whittles or a desecration.
