I regret to inform you that you're basically cooked for all content in the setup you currently have. 5k total hit pool is incredibly low, by like 2x. Going a comet CoC build without some very OP resource generation is a crapshoot, most pathfinder builds solve this right now with the unique chest that gives spell leech ( think it's called robe of the covenant or something like that )

You have basically completely sub par gear on every slot, you are using a unique ring and a lvl 34 ring it seems like you prolly picked it up in act 3 and never upgraded, your wand is bad amulet needs upgraded and i don't think you're going to realistically be able to do a shrine sceptre for t15+ content. Vertex is +2 when even a +3 is relatively cheap and a +4 is attainable, your passive tree is kind of a mess with a bunch of really low value additive damage and ev/es nodes when almost anything you chose would have been better
