Since you already have a clear endgame of long-form youtube, start on instagram reels, not youtube shorts. The algorithm there is way more forgiving for new accounts and the medical + lifestyle combo actually does really well because you can hit both the wellness crowd and people who like that "day in my life" content at the same time.
The other thing is instagram makes it easier to build a personal connection through stories and DMs while youre still small, which matters a lot for a personal brand. Once you hit a decent following there, the jump to long-form youtube is so much smoother because youre bringing an audience with you instead of starting from zero twice.
