Do I write my username then “TikTok Proof of Age” right after it?

When I do submit the photo, do I put my username on the piece of paper and then “TIkTok Proof of Age” right after it and then my birthdate and do I have to be in the selfie with my guardian? Also do I submit the photo in the chat when I do it?
