We had an accidental stick during my EMS class waaaay back in like 2001ish, and we got to work him up and everything before we threw him in a meatwagon to go to the hospital.
He never lived it down lol, the instructor was literally saying "now that's a live epipen, so make sure you don't--"

I almost died from an allergic reaction and was at the ER and the doctor gave me an epi-pen shot. It was the most painful shot I've ever gotten in my life. Holding that huge steel needle in your leg for ten seconds is horrible!
As an ex first responder for twenty years this would be the one guy on shift I would avoid working with at all cost.
I took three of those in the ER after finding out I’m anaphylactic to penicillin.
I don’t remember it hurting.

I take a immunosuppressant every couple of weeks and the injection is hardly painful at all; are epipen needles that much larger? Anyone know why if so?
