You are SOL in terms of having the App on your phone if you swap phones. Tiktok is not on either Apple or Googles App Store.

The Apple CEO is following the law passed by Congress and held up by the Supreme Court that forced them to remove Tiktok.

The only way I know around the download part of the TT ban that’s still being taken seriously is changing the country location of your Apple, Google, or other email/App Store account and use a VPN. I strongly recommend not having any active subscriptions directly through your Apple/iCloud (EX: Apple Music,Apple TV) as they will prevent you from changing your country/region to for the duration of that subscription
