IP Report Form Broken / Bugged

I’ve been trying to use this report form with absolutely zero luck for the past 3 days. Has anybody else noticed this? Even after verifying my email (and seeing “Email Verified!”) it kicks me back to the start of the form every time. I also don’t receive a case number email when using the impersonation form. Multiple browsers and devices.
