I liked and followed the pages of a few obviously "challenged" people (down syndrome, severe autism etc) and now I get videos like this all the time. How is the algorithm so advanced to know who is challenged just from a video without any hashtags?

well your interaction got you identified as being part of the community of people with the weird kink of watching these types of videos and so now you get what they all, as a group, also like.
Of course they like all different/incompatible things but the type of video they nearly ALL like might be this type of intellectual disability video, so whatever this club finds from now on will now be tried in your feed, too.
