Cant follow anyone

Had my account for around three years now and I had this problem once when I had multiple accounts but I got rid of my other account a while ago and the problem stopped however it's come back for no reason.

When I try to follow someone, wether or not its a private account it says I'm following them but when I refresh I am not, and it won't stop doing this. It doesn't matter what account it is, it always happens. I have tried clearing cache, restarting my phone but neither has worked.
