Copyright infringement

I keep trying to report a video of mine that was stolen off my Tik Tok account and posted on instagram. It had blown up a lot on TikTok and is now doing so on their insta post but they’ve not credited me at all, changed my caption and are using the video to sell product. Ive filled the form but it keeps crashing and saying it’s not working. Very frustrating…
